Nipple Pain While Breastfeeding: Real Causes and Solutions

Cracks, poor latch, thrush, or vasospasm — how to identify the problem and solve it without stopping breastfeeding.

Nipple pain is the number one reason for early breastfeeding abandonment. And most of the time it has a solution — if you know what's causing it. The first 2 weeks can hurt as the nipple adapts, but intense pain, bleeding cracks, or persistent burning always have a treatable cause.

Diagnosis by type of pain

Pain only at the start of feeding

Likely cause: poor latch. Baby only takes the nipple, not the areola. Solution: verify the baby's mouth covers 2-3 cm of areola, not just the nipple.

Pain throughout the feeding

Likely cause: short frenulum (tongue tie) in the baby. Baby cannot extend tongue enough. Solution: request frenulum assessment from pediatrician or lactation consultant.

Burning after the feeding

Likely cause: thrush (yeast infection). Nipple looks pink, shiny, with itching. Solution: antifungal treatment for both mother and baby (mouth) simultaneously.

White/blue pain after finishing (color change)

Likely cause: Raynaud's vasospasm. Cold causes vascular spasm. Solution: warm nipple immediately after each feeding, avoid cold.

Visible cracks that bleed

Blood in milk is not dangerous for the baby. Treat the crack with pure lanolin or breast milk itself (antibacterial). See a lactation consultant.

What DOES work for the pain

  • Pure lanolin (without water) on the nipple after each feeding — best evidence for cracks

  • Breast milk on the nipple: has antibacterial and healing factors

  • Nipple shields between feedings so the nipple doesn't rub against clothing

  • Do NOT use soap on nipples — it removes natural lubrication from Montgomery glands

  • Change feeding position: Football hold, Biological, Side-lying — different pressure, different pain

If you have fever, hot and hard breast, streaky redness, it may be mastitis — requires antibiotics and yes, you can continue breastfeeding during treatment.
